Thomas J. Callahan, MA, LPC, CAADC, CCS – Regional Director
Tom has been with the White Deer Run/Cove Forge Behavioral Health Systems for over 13 years, and currently holds the position of Regional Director.
He has spent three decades working with individuals and families who have been affected by addiction and mental illness. Tom has developed both inpatient and outpatient models for treating co-occurring disorders and is responsible for developing the model currently used at Cove Forge. His journey has taken him through every level of care, as he has been in private practice since the early ‘90s and has served as a Field Placement Supervisor to many students entering the field. Tom is committed to contemporary, integrated Disease Model Treatment focusing on hope and present day change.
Tom holds a Master’s degree in counseling and is licensed to practice in the state of Pennsylvania. He has also been certified as an addiction’s counselor, a co-occurring disorders counselor, and a clinical supervisor. He has provided trainings and lectures across the state. In 2010, Tom was awarded the Northeast Recovery Division Core Values Award and in 2012, he received the award for Executive of the Year Residential Eastern Recovery Division.
Anita Stoudnour, RN – Assistant Administrator
Anita graduated from a diploma program at Memorial Hospital in Cumberland, Maryland and Frostburg State College and has been a Nurse Supervisor/Manager for over the past 40 years. As long as she can remember, Anita has been dedicated to serving others, choosing a career in healthcare in order to make that difference. Her varied healthcare experiences are in hospital, nursing home, homecare, occupational health, marketing, administration, and health insurance. Anita is able to make a difference assisting the Regional Director and supervising the nursing departments, environment of care & facility operations, medical records, risk management, administrative support and all quality improvement activities.
Donna Bookhammer, CADC, CCDP, ICCDP, CTRTC – Adult Clinical Program Director
Donna has been employed with Cove Forge Behavioral Health Systems for over 24 years. She worked as a Counselor and Mental Health Counselor for eight years, as well as the Family Program Director for four years. Donna has been the Adult Clinical Director for 13 years and has remained involved with the development and implementation of the Dual Diagnosis Program and the H.O.P.E (Heroin, Opiate, Positive Experience) Program. Donna is certified by the State of Pennsylvania as a certified Co-Occurring Disorder Professional, is a Certified Addictions Counselor and is also certified through the Glasser Institute in Choice Theory/Reality Therapy.
John T. Symons, MD – Regional Medical Director
Dr. Symons received his medical degree from the University of Maryland School of Medicine in 1979 and completed a three-year residency in Family Practice at Altoona Hospital (now UPMC Altoona). After two years in private practice, Dr. Symons returned to the Altoona Family Practice residency program as part of the faculty, eventually becoming Program Director. After leaving the residency program, Dr. Symons served as Medical Director of SCI Rockview from 1998 to 2012. He started work at Cove Forge Behavioral Health part time in 2000 and became the full time Medical Director in 2012. Dr. Symons is certified by the American Board of Family Practice and the American Board of Addiction Medicine. He is also a member of the American Society of Addiction Medicine.
As Medical Director of Cove Forge Behavior Health, Dr. Symons spends the majority of his time in direct patient care with emphasis on the detox units and on medically challenging patients in the rehab community. The remainder of his time is devoted to supervision of the mid level practitioners, medical screening of admissions with serious medical needs, utilization management, records review for quality of care purposes, participation in multi-disciplinary team meeting and participation in management meetings as needed.
Larry Breedlove, MS Psychology – Counselor, Music Program
Larry has been a counselor at Cove Forge Behavioral Health Services since 2010, and has been playing music for 20 years.
He began his first band at the age of 18 in the town of Albuquerque, New Mexico. Larry took three classes in music at the University of New Mexico, and played in over three bands — all playing locally and getting some airplay on local radio.
In 2000, Larry moved to Los Angeles and began studying music at Musician’s Institute. His focus of study was ensemble playing, private lessons, theory, and learning different styles of music including Latin Jazz, Hip Hop, Rock, and Traditional Jazz.
Eleven years later, Larry began a music practice/performance group at Cove Forge. This group consists of clients who are currently in treatment. They work together to perform music and display their talent. Larry utilizes this group to help those involved work on developing teamwork that will alleviate stress, display talent, and work towards set goals. In addition to this group, Larry also conducts the Drum Circle Group at Cove Forge.
Carly Haught, BA, RN – Director of Nursing
Carly obtained her bachelor’s degree in Clinical Sociology at Indiana University and her Registered Nursing license from Penn State University. She has been working in the drug/alcohol and mental health field for 14 years. She started her career in admissions and then spent many years as an outpatient counselor. Carly began working at Cove Forge in 2006 as a floor nurse, and shortly thereafter she became the Detox supervisor in 2008. In 2011, Carly took on the role of the Director of Nursing. Along with the supervision of nurses, she also supervises the detox technicians and detox counselors.
M. Wendell Davis – Clinical Technician Supervisor
Wendell is a graduate of Mt. Aloysius College and holds an Associate degree in Liberal Arts. He was certified by the William Glasser Institute in Reality Therapy/Choice Theory, and has 20+ years of experience in the field of Human Development. In 1997, Wendell was employed by Charter Behavioral Health, now known as a division of White Deer Run at Cove Forge in Williamsburg, PA. Wendell’s expertise including helping to create a free style voluntary group, helping clients to understand that change is inevitable and survival is optional, confronting daily personal issues ranging from the mundane to severe, and more. As the Clinical Technician Supervisor, Wendell’s focus is on providing education and training to the Clinical Technicians in many areas, including our H.O.P.E. Program. With Wendell’s experience and his motivational speaking skills, he is an inspiration to his staff and clientele in conducting groups and staff meetings.
Joel Last, MD
Dr. Last is a psychiatrist who is Board Certified by the American Board of Psychiatry & Neurology and has been practicing Psychiatry in Western Pennsylvania since 1991. He has his own full-time practice and has for the past ten years. Prior to that, Dr. Last has been affiliated with Excella Health, and has been on staff at Latrobe, Westmoreland, Jeanette, and Frick Hospitals. He has also served as the Director of Psychiatric Services for over ten years at Cove Forge.
Dr. Last uses natural remedies as well in his approach to wellbeing and recovery. He has used the Serenity Prayer as the basis for helping clients to deal with anxiety and depression. He has over 18 years of experience in treating adolescents and those with drug and alcohol/co-occurring issues.
Dr. Last completed his Residency in Psychiatry at Georgetown University Medical Center in Washington, D.C. in 1990. His training in Psychiatry spans the spectrum form the biological approaches to the treatment of mental health, to the Freudian based psychoanalytic approaches. Prior to completing medical school, Dr. Last was a practicing Pharmacist for three years at Children’s Hospital in Detroit, Michigan, which gives him an excellent background in the use of medications.
Andrew Helmer, MBA – Operations Assistant
As the Operations Assistant at Cove Forge, Andy has been with Cove Forge for 15 years. In addition to managing Finance, Risk and Safety Management, and being a certified trainer for their Choice Theory/Reality Therapy Program, Andy is also the ROPES/Activities Coordinator. Through our experiential therapies clients can work on improving trust, self-esteem, endurance, problem solving, and communication.
